In order to access Vibrant Care Pharmacy online services, you will be asked to sign in with a user name and password, which we refer as your credentials. As part of creating your credentials, you may also be requested to provide questions and secret answers, which helps in verifying your identity and assists in resetting your password, as well as an alternate user name. We ask you to provide personal information, such as your e-mail address, name, home or work address or telephone number. We may also collect demographic information, such as your ZIP code, age, gender, preferences, interests, and favorites. If you choose to make a purchase or sign up for a paid subscription service, we will ask for additional information, such as your credit card number and billing address that is used to create a Vibrant Care Pharmacy billing account.

Security of Your Personal Information Vibrant Care Pharmacy is committed to protecting the security of your personal information. We use a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, or disclosure. Get a list of those with whom we’ve shared information

  • You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why
  • We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

Your Choices

How do we typically use or share your health information?
